This facility is seeking a bilingual nurse practitioner to work as part of a disciplinary team caring for the elderly population. This community-based healthcare program follows the PACE model. The new NP will have a patient panel and will manage their schedule according to patient needs. This is outpatient weekdays with call and some patients that require home visits. This position is open to both new and experienced NPs who have experience or a desire to work with the frail and elderly. Adult geriatric or family board certification and state license required.
