Providers Typical Day:

·        On site supervision for compliance, checking SRS's, on treatment visits, occasional follow up or Consult.

·        Cases - External beam treatments, OTV's and follow up procedures required

·        25-30 ppd

Requirements:

·        Illinois License Required

·        Minimum 1 Year Experience

·        DEA Required

·        BLS Certification

·        CSR License
