Job Description:

• Board Certified in Family Medicine
• 20 patients per day
• Must have or be willing to obtain the suboxone waiver, required to acquire and maintain certification for Basic Life Support for Healthcare Professionals no later than 90 days of hire, must be able to pass a respirator fit test at time of hire and on an annual basis
• Office staff includes: Medical Director, Psychiatric NP, Primary Care NP, Medical Assistant, Patient Care Coordinators, Dietitian Nutritionist, front desk staff, and behavioral health staff
• Provide whole person healthcare from infants to older adults in an integrated care setting
• The physician will diagnose and treat acute and chronic illness, provide routine screening and counseling on lifestyle changes in an effort to prevent illness
