Job Details

Shifts include: Monday - Friday 8:00a - 4:30pp, On average, there are seven (7) on-call days (evenings/weekends) with weekend telephone triage.
Duties entail: annual assessments, Post-hospital visits, Acute visits
Minimum 1 year working with the frail or elderly population
MA license in hand - Required
They are a Program of All-inclusive Care for the Elderly, tightly coordinated care and support designed to help seniors continue to live safely at home and avoid moving into a nursing home.
