Locum Physician Assistant (PA) - Geriatrics in New York - We are seeking an experienced Nurse Practitioner or Physician Assistant for a Locums assignment to convert to Perm (with in 3-6 months) at a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care. Compensation: $75 – $95 per hour Position Details: * Schedule: Full Time Hours Guaranteed * Monday – Friday - 5 Days Per Week * Patient Load: LOW DAILY CENSUS * Home Care: 3–5 patients per day * Clinic: Small census of 7–10 patients per day * Setting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients * Locations: Bronx, Brooklyn and Ridgewood, NY Responsibilities: * Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients * Diagnose, treat, prescribe medications, and develop individualized treatment plans * Order and interpret diagnostic tests (labs, imaging, etc.) * Document accurately in the EHR system * Provide patient education and promote wellness initiatives * Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care * Refer patients to specialists and manage follow-ups * Ensure compliance with state, federal, and professional regulations Requirements: * Active NP or PA license in NY * Minimum 2 years of primary care experience * National Board Certification * Spanish Speaking Preferred * Expedited Credentialing * Medicaid and Medicare Provider Id required
