Locum Nurse Practitioner (NP) - Family Practice in Des Moines, IA - Primary Care Nurse Practitioner | In Home Health Assessments Location: IA Start: ASAP to ongoing Setting: In Home Assessments Statewide Opportunities: Opportunities are available throughout most counties statewide. Assignments are scheduled based on provider availability and county demand, with opportunities in all but a limited number of restricted counties. Travel and Lodging: Airfare, rental car, and hotel accommodations are provided based on provider availability. Position Overview We are seeking experienced Nurse Practitioners to perform in home health and wellness assessments for geriatric patients. This is a flexible, provider driven opportunity built for independent clinicians who want control over their schedule while delivering meaningful, patient focused care. Schedule and Volume * Typical schedule: Monday through Sunday, 7:00 AM to 7:00 PM * Daily volume: 5 to 8 in home assessments per day * Availability requirement: Providers must be available at least two full days per week (not necessarily consecutive) and include one weekday * Weekend only availability: Not accepted Clinical Responsibilities * Health and wellness assessments including history and physical evaluations * Medication reviews * Vital signs * Point of care screenings as applicable (finger stick, urine sample) * Fecal kit distribution as applicable Patient Population * Ages: Geriatrics Incentives * Weekly incentive available for meeting visit goals * Flexible, self directed schedule designed to help you choose when and where you work Credentialing Credentialing is self paced and typically takes 30 to 45 days. Locum Physician (MD/DO) - Family Practice in Iowa - Primary Care Nurse Practitioner | Tennessee Location: Tennessee Schedule: Ongoing per diem day shifts Openings: 1 Position Overview * Outpatient corporate clinic coverage providing acute care, wellness, and primary care services for employees and eligible family members. * Focus on wellness and prevention, acute episodic care, and chronic condition management. * May include occupational health and work related injury evaluation and return to work determinations as needed. Clinical Scope * Ages seen: 2 years and older. * Patient volume: up to about 3 patients per hour (site dependent). * Workers’ compensation and occupational health services may be included (site dependent). * No pain management. * EMR: Athena (experience preferred; training provided).
