Requirements Blanchard Valley is looking for a locum Vascular Surgeon to start as soon as possible. Schedule: • Open to start date • This would be a part-time position with us requesting monthly coverage of 1 week/month 7a Monday-7a Monday or 2 weekends per month 7am Friday-7am Monday • There is no expectation that this locum would see scheduled patients in clinic but would only cover inpatient and ED consults and call coverage during this time. Call Requirements: • Clinic Call: No • Inpatient Call: Yes • Rounding required: Yes • Called in vs. phone consults: Infrequently • Call start and end times: 7a-7a Procedures: • This physician would not schedule any elective procedures but would cover any urgent/emergent procedures picked up on call. Patients: • Peds: No • Types of Cases: Urgent/Emergent vascular cases Other: • Support staff is available to them: OR team • Sole provider: Covering time off for the employed vascular surgeon • Reason for additional locum coverage: To provide time off coverage for the employed vascular surgeon • Case logs required: Yes Would need the last two years Information I want included in the presentation Presentations that do not include this information will not be reviewed: State licensure Board Certification Status Malpractice information Have they had any action on licensure? Availability Travel requirements Rates Have they ever been terminated? Why are they doing locums? Have they worked a locums assignment before? Why did they leave their last place of employment? Explanation of any gaps 2 months or more within the last 10 years or since completion of training Case logs from last two years Name Clear/Presentation Process: Then there are two options: If there is a candidate that meets some but not all criteria: Send me a summary of the candidate without their name and what is in question. I will run it past the director to see if that is something they are willing to consider. If it is decided that it is something they will consider send a name clear. If cleared send a full presentation. If they meet our need exactly go ahead and send a name clear. If cleared send a full presentation. If anyone is presented to me that does not meet the specifics of the position and was not questioned beforehand we will not be responsible for paying any associated fees such as a buyout. Ownership would not apply at that time since the candidate didn’t meet the need we were looking for. If there is ever a question on if a candidate has given you permission to present them for this specific search I will require written documentation showing it. If you cannot produce this ownership does not apply. Malpractice Insurance Requirements: Maintain continuously in force professional liability insurance with individual limits of not less than 1000000 per occurrence and 3000000 annual aggregate. Must be with a licensed insurer rated B or better by A.M. Best Company or it’s successor. Travel Expenses: Depending on the travel that is required we may place caps on travel costs. Lodging Policy: Group shall provide reasonable lodging for provider during the term of this Confirmation for so long as he/she lives more than one 1 hour away and one of the following applies: 1 filling two consecutive days of shifts ex. Monday and Tuesday or 2 completes a shift after 8pm. Exceptions may be made for weather etc. but must be approved prior to expense. There is a cap on what BVHS will be charged. It is 130/night including all fees and taxes.
