Requirements Locums Petaluma, CA Hospitalist Night Shift Structure: Night shift differential of $25.00/hr to be added to any night shifts scheduled and worked. We will REQUIRE a clean file for this search in order to work September dates as listed. For a clean file: • A "clean" applicant o Physicians with large settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges • The RFA, CV, and 24 months of relevant activity • A completed application separate from / following the above step • References on standby to respond ASAP Looking for 2-3 providers. ______________________________________________________________ SCHEDULE • Coverage Type: Inpatient • Schedule Type: Hospital • Call Type: N/A • Dates of Need: Day: 9/1, 9/6, 9/13 Nights: 9/1, 9/4-8, 9/10, 9/13, 9/15-17, 9/18-23, 9/26-28, 9/30 • Shift Times: 7am-7pm or 7pm-7am • Guaranteed Hours per Day: 12 • Hours Worked for OT to apply: 12 per day • Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday OT Policy: Overtime must be pre-approved by medical director by end of shift by text or email. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active only • DEA Required: Yes • Boards: BC or Board eligible within 3-5 years of residency Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. ________________________________________________________________ ADDITIONAL DETAILS Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): • NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return. • WILL be reimbursed: o Airport parking at provider’s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips
