We prefer local candidates but need to understand what travel will be required if not. Ensure they are proficient with EPIC or EMR experience in general and indicate in this section as well. If they are familiar with the High Desert area, please also note that on the presentation. Provider will need to submit some application for payor credentialing (CPPA, drivers license, social security card, MD Masters and Bachelors degree certificates, medical board of CA (wall certificate or pocket card), DEA, current malpractice document. Please start to gather these so we can get the provider started asap. ________________________________________________________________ SCHEDULE • Coverage Type: Outpatient • Schedule Type: Clinic only • Call Type: N/A • Dates of Need: Full time need • Shift Times: 8am-5pm • Lunch N/A unpaid • Provider Min # Shifts Required: 4 weeks per month • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 9 per day • Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active only • DEA Required: Yes • Boards: Board Certified or Board Eligible within 5 years of Residency • Skills/Certs: BLS ____________________________________________________________________________________ Additional Details: We are looking for one provider to cover a full-time need at our St. Mary's High Desert- Pediatrics location. Provider will not need privileges so they can start once access is set up. Physician must be available to work 5 days a week, 4 weeks a month (Monday- Friday). Scheduled hours will be 8am-5pm each day, provider may take a lunch when they see fit in those hours. No call just clinic. The patient count per day is an average of 25 patients but it could fluctuate. This area does have a high no show rate, so they do schedule to account for if a patient does not make their appointment. We would look at candidates that are interested in locums to perm as well. If the provider and clinic find it to be a mutual fit they will look to make the transition. High Desert Demographic: o High demand clinic, socioeconomic challenged community.
