Requirements Locums Seattle, WA Hematology Oncology Update 7/17: 1) Please only submit providers available for full-time support. Schedules that are 2 weeks on and then 2 weeks off or similar are not helpful for our department in its current state. ________________________________________________________________ SCHEDULE • Coverage Type: Outpatient (Some inpatient call on occasional weekend) • Schedule Type: Clinic and Call • Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call • Dates of Need: Full time need • Shift Times: 7:00am-5:00pm • Lunch 30 minutes-1 hour unpaid • Provider Min # Shifts Required: 4 weeks per month, full time coverage need • Guaranteed Hours per Day: 10 • Hours Worked for OT to apply: 10 per day • Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active WA or Active IMLC • DEA Required: Yes • Boards: BC only • Skills/Certs: • Other: Lymphoma experience and cell therapy experience would be a plus ________________________________________________________________ ADDITIONAL DETAILS We are needing full time coverage from one experienced Hematology Oncology provider. Our vital coverage need begins in October, we could start a locum earlier if privileges are issued sooner. Providers will be scheduled for 4 10 hour shifts per week, Monday-Thursday 7a-5p. Clinic is closed daily 12-1pm for lunch hour. Providers will be on call for established Hem/Onc clinic patients 1-2 nights per week. Providers will also participate in a shared weekend call pool for oncology patients in the hospital, weekend call is shared between all Oncology providers (15-20 total providers) so providers will only be on weekend call very few weekends throughout the year.
