• Licenses: Active Colorado Medical License, Active DEA with Colorado address
• DEA / CDS: Active DEA required
• Certifications: ACLS, ATLS, PALS, BLS required
• Other Qualifications: CME credits past 24 months, case logs, CAQH number

• Provide primary care and family medicine services to patients.
• Perform minor procedures including joint injections, toenail removals, and lesion removals.
• Manage patient volume of approximately 20 patients per day.
• Work Monday through Friday, 8am to 5pm, with admin time.
• Collaborate with physicians and other healthcare staff as needed.
