About the Position

Position Overview

We are seeking a Board Certified or Board Eligible Physician to join a growing team providing high-quality primary care services to patients in nursing home and skilled nursing facility settings. This role offers the opportunity to deliver comprehensive, patient-centered care while collaborating with an interdisciplinary healthcare team dedicated to improving patient outcomes.

The ideal candidate is committed to delivering compassionate, evidence-based care to geriatric and long-term care populations, with a focus on diagnosis, treatment, prevention, and chronic disease management.

Key Responsibilities

  • Provide comprehensive primary care services to patients in nursing home and long-term care settings.
  • Perform patient evaluations, assessments, diagnoses, and develop individualized treatment plans.
  • Order and interpret laboratory, imaging, and diagnostic tests.
  • Prescribe medications and treatments in accordance with evidence-based clinical guidelines.
  • Refer patients to specialists when medically appropriate.
  • Educate patients and caregivers regarding diagnoses, medications, treatment plans, and lifestyle modifications.
  • Collaborate with nurses, advanced practice providers, therapists, and other healthcare professionals to ensure coordinated patient care.
  • Maintain accurate, timely, and compliant medical documentation.
  • Adhere to federal, state, and organizational policies related to patient safety, infection control, quality improvement, and HIPAA compliance.
  • Participate in quality initiatives designed to improve clinical outcomes and patient satisfaction.

Qualifications

Required:

  • MD or DO from an accredited U.S. medical school (or equivalent pathway meeting U.S. requirements).
  • Board Certified or Board Eligible in one of the following:
    • Geriatric Medicine
    • Internal Medicine
    • Family Medicine
    • Palliative Care/Hospice
  • Successful completion of an accredited U.S. residency program (or equivalent qualifying pathway).
  • Active New York medical license or eligibility to obtain one.
  • DEA registration (or eligibility to obtain during credentialing).
  • Strong clinical, communication, and documentation skills.
  • Ability to work collaboratively within a multidisciplinary healthcare team.

Preferred:

  • Experience providing primary care in nursing homes, skilled nursing facilities, long-term care, or outpatient settings.
  • Experience managing geriatric and medically complex patients.
