About the Position
Position Overview
We are seeking a Board Certified or Board Eligible Physician to join a growing team providing high-quality primary care services to patients in nursing home and skilled nursing facility settings. This role offers the opportunity to deliver comprehensive, patient-centered care while collaborating with an interdisciplinary healthcare team dedicated to improving patient outcomes.
The ideal candidate is committed to delivering compassionate, evidence-based care to geriatric and long-term care populations, with a focus on diagnosis, treatment, prevention, and chronic disease management.
Key Responsibilities
- Provide comprehensive primary care services to patients in nursing home and long-term care settings.
- Perform patient evaluations, assessments, diagnoses, and develop individualized treatment plans.
- Order and interpret laboratory, imaging, and diagnostic tests.
- Prescribe medications and treatments in accordance with evidence-based clinical guidelines.
- Refer patients to specialists when medically appropriate.
- Educate patients and caregivers regarding diagnoses, medications, treatment plans, and lifestyle modifications.
- Collaborate with nurses, advanced practice providers, therapists, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate, timely, and compliant medical documentation.
- Adhere to federal, state, and organizational policies related to patient safety, infection control, quality improvement, and HIPAA compliance.
- Participate in quality initiatives designed to improve clinical outcomes and patient satisfaction.
Qualifications
Required:
- MD or DO from an accredited U.S. medical school (or equivalent pathway meeting U.S. requirements).
- Board Certified or Board Eligible in one of the following:
- Geriatric Medicine
- Internal Medicine
- Family Medicine
- Palliative Care/Hospice
- Successful completion of an accredited U.S. residency program (or equivalent qualifying pathway).
- Active New York medical license or eligibility to obtain one.
- DEA registration (or eligibility to obtain during credentialing).
- Strong clinical, communication, and documentation skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.
Preferred:
- Experience providing primary care in nursing homes, skilled nursing facilities, long-term care, or outpatient settings.
- Experience managing geriatric and medically complex patients.
