Requirements Sept 8-11 Sept 17-18 Sept 24-25 Oct 1-2 Oct 19-30 Dec 28-31 Specialty: Electrophysiology MD Shift Details: Monday-Friday 8a-5pm Schedule: Monday-Friday 8a-5pm no call required Work Details: • Support Staff: 1 assigned clinical team out-patient 1 assigned APP in-patient • Amount of Coverage Needed per Month: up to 40 hours per week once per month and as needed for vacation coverage • Patients per day: 16 OUT-PATIENT 10-15 IN-PATIENT • Patient Population/Age: 18-100 • Department Description: Electrophysiology • EMR: EPIC • Hospital Bed size: 270 • Electrophysiology: Pacemaker Implant ICD implant traditional and sub-Q Loop recorder implant and/or explant Ablations: AVN SVT AFLUTTER • Skills Required: Cardiology/Electrophysiology Practice Setting: select as many as needed • Academic/Training: Yes • Clinic/Private Practice: Yes • Hospital: Yes Requirements: • Board Certification: BC or BE • License Required: active PA or IML CLOQ required to submit • DEA: Yes • Certifications Required: ACLS/BLS • Minimum Experience Required: Credentialing Timeframe: 90 days temps available with clean file 2 hours gratis for 24 hour call Active PA License or IMLC LOQ in hand Required fixed Shift Info: Day 5x8-Hour 08:00 - 17:00
