Requirements

• Board Certified or Board Eligible in Plastic Surgery
• Active Pennsylvania license or IMLC Letter of Qualification (LOQ) required for submission
• Active Pennsylvania DEA
• Minimum of 5 years of experience
• Expedited credentialing available (less than 90 days)
