Requirements **Required for presentation: must have actual state license board certification in hand, and active DEA for the state of application and CSR if needed per state in hand.** Education/Training: Must have completed 3-year residency in EM, FM or IM w/ Moonlighting approval Clinical Experience: Must have over 2000 contact hours within last 2 years in ED or Acute Care. License: Active unrestricted MS medical license and DEA w/ MS address & Schedules 2,2N,3,3N,4 & 5 Board Certification: Board Cert or Board Eligibility EM, FM or IM Merit Badge: ACLS, ATLS and PALS if not BC in EM or EM trained. Immunizations: Flu required | COVID preferred , not required Case Log: Documentation of case logs for past 2 years. CME/CEU: Proof of 40 CME hours over the past 2 years. Specific Facility Procedures: Moderate Sedation Malpractice Coverage: $100K/$300K Claims Made Coverage Additional Details: Required to be a supervising or backup physician for NP's and PA's.
