• Monday - Friday schedule, 7:30 am - 5 pm with shared after-hours phone call
• Hospital-employed position with outpatient-only clinical setting
• Small patient panel of 20 patients supports individualized, high-quality care
• Lead a dedicated care team including medical assistant, care facilitator, and care coordinator
• On-site services include digital X-ray, ultrasound, stress tests, Doppler, Echo, EKG, and spirometry
• Associate Center Clinical Director role blends direct patient care with clinical leadership
• Responsibilities include mentoring and coaching primary care providers within the center
• Board-certified geriatric medicine physician required; DEA required
