Badges: ACLS, ATLS & PALS (ATLS only needed if not ABEM MD/DO - Education/Training Requirements Residency Completion in EM , FM or IM MD/DO - Clinical Experience Requirements New Grad Accepted MD/DO - License Requirements Active unrestricted LA Medical License, DEA License w/ LA address, LA Controlled Substance & Supervising Physician License (PA supervision) MD/DO - Board Cert+N2ification Requirements BC in Emergency Medicine, or Internal Medicine or Family Medicine MD/DO - Merit Badge Requirements If ABEM-certified & actively participating in ABEM MOC no merit badges required. . MD/DO - Immunization Requirements Flu (within current flu season) | TB (past 12 months or Chest X-Ray with positive TB Vaccinations | Covid vaccine MD/DO - Case Log Requirements Documentation of case logs for past 2 years. MD/DO - CME/CEU Requirements Proof of completed CME's for the last two years MD/DO - Facility Procedure Requirements MSO sends out sedation modules providers must complete during onboarding and during reappointment MD/DO - Malpractice Coverage Requirements "$100K/$300K Coverage plus enrollment in LPCF (locums - must be enrolled a minimum of 90 days at a time)" MD/DO - Additional Details Must meet supervising physician requirements for NP/PA oversight.
