A federally qualified community health center in Levy County (northwest coast of Florida) is adding a family medicine physician to a site in Orange Park, FL, a location just 30 minutes south of Jacksonville, FL.

Practice Highlights

• 100% outpatient primary care with a Monday-Friday schedule
• Joining 2 experienced NPs
• Split time between Orange Park and Fleming Island offices.
• Average of 18-20 patients per day
• Smart Scheduling with roughly 70% of visits pre-scheduled
• NO visa support
