Requirements Please have provider's SSN DOB ready at time of offer as it is needed to process their file in a timely manner once offered the position. *** Include 2hrs of gratis for 24hr weekend call Type of privileges- Hospital Clinic Setting- Inpatient # beds 21 Rounding Admissions- Yes both Patient Demographics- Adults Peds Average patients per day - Hospital 2 / Clinic 12-20 Required procedures- Ortho Trauma within scope of privileges Require Clinical activity in past 24 months- Applicant must provide documentation case list of provision of clinical services 100 patients representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed. Licensed - Yes Oregon license in hand at time of presentation is required. Working Solo? - YES Required certifications: BLS EMR: EPIC – MUST have recent experience working in EPIC within last six months Shift Info: Day 5x8-Hour 08:00 - 16:00
