About the Position
🏠 Primary Care Physician – House Calls (Leadership Track)
📍 Location: New York City & Surrounding Counties
🏢 Facility Setting: Home-Based Primary Care / House Calls Program
🕒 Employment Type: Full-Time Preferred
💰 Compensation: $280,000 – $300,000 Annually (Base + Performance Incentives)
✨ We are seeking Board Certified or Board Eligible Primary Care Physicians (Internal Medicine or Family Medicine) to join a dynamic house calls program delivering high-quality, in-home care to medically complex, homebound patients. This role also offers a clear pathway to a Medical Director leadership position, providing strong autonomy, growth, and impact.
🎯 Qualifications:
✔ MD or DO from an accredited U.S. medical school
✔ Completion of Internal Medicine or Family Medicine residency
✔ Board Certified or Board Eligible in Internal Medicine or Family Medicine
✔ Active New York State medical license
✔ DEA registration
✔ Minimum 5 years of post-residency clinical experience
✔ Strong background in primary and diagnostic care

• Geriatric or palliative care experience
• Experience managing homebound or medically complex patients
• Strong leadership and team collaboration skills

Requirements
🔹 Key Responsibilities:
✅ Provide comprehensive primary care services in home-based settings
✅ Perform detailed patient assessments including history, physical exams, and medication review
✅ Diagnose and manage acute and chronic medical conditions
✅ Prescribe medications and coordinate treatment plans
✅ Collaborate with interdisciplinary teams including APPs, nurses, and care coordinators
✅ Educate patients and families on disease management and lifestyle care
✅ Maintain accurate and timely clinical documentation
✅ Coordinate referrals, labs, home health, and therapy services
✅ Supervise and mentor interdisciplinary care team members
✅ Support administrative and leadership functions with progression to Medical Director role
