• ABOS Board Certified or TRULY Board Eligible in Total Joint / Adult Reconstruction Orthopedic Surgeon (must become certified within 5 years) - [REQUIRED] * no exceptions
• Fellowship trained in Adult Reconstruction / Total Joint Arthroplasty [PREFERRED]
• Valid IL & IA license or Active IMLC Willing to Obtain License - [REQUIRED]
• Strong clinical and surgical skill set with a passion for total joints and revision work - [REQUIRED]
• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - [REQUIRED]
• DEA - [REQUIRED]
• BLS Certification and maintain recertification - [REQUIRED]
• Clean malpractice and license history - [HIGHLY PREFERRED] / [PREFERRED]
• Does the provider require visa support - No J-1 or H-1B (Note in Additional Submission Details) - [REQUIRED]
• Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - [REQUIRED]

• $739K guaranteed (2 years); production after 10,650 wRVUs
