About the Position

We are seeking an experienced Nurse Practitioner to provide primary care services within assisted living communities throughout the Albany region of New York.

This role is ideal for an NP who enjoys caring for the geriatric population and values developing long term relationships with residents. The provider will focus on preventive care, chronic disease management, routine follow ups, and comprehensive primary care services.

Schedule Options

Part Time

• 2 to 3 full days per week

• Coverage between Hoosick Falls, NY & Cambridge, NY

Full Time

• 4 to 5 full days per week

• Coverage between Hoosick Falls, NY, Cambridge, NY & Troy, NY

Requirements

Required

• Active New York Nurse Practitioner License

• Active DEA Registration

• Currently enrolled and credentialed with Medicare & Medicaid

• Minimum 3,600 clinical practice hours required for independent practice in New York

• Primary care experience

Preferred

• Geriatric medicine experience

• Assisted living experience

• Long term care experience
