Requirements Start- ASAP must have WA license in hand at time of present Type- APC/APP NP is ideal due to work without a supervising physician • Yearly refills • Diagnose and treat erectile dysfunction • Kidney stones • Male incontinence • Cystoscopies • Surgical assist Why Open?- Gap in coverage Working at Medical Center and Urology Clinic 1 mile away Both Hospital and Clinic Privileges Needed Inpatient Outpatient Setting Bed #- 122 Call 7 days of unrestricted call per month Demographics- 17 Avg. Patients per day- 18 Procedures- standard urology procedures in the clinic and operating room Required clinical activity in the last 24 months case logs- Applicant must provide documentation case list of provision of clinical services representative of the scope and complexity of the privileges requested during the previous 24 months. Will provider work solo? If not other providers specialties working with them- Yes Licensed Candidate: Yes candidate must have a Washington state license in hand at time of present. Required certifications Must be from American Heart Association or American Red Cross as applicable- BLS and ACLS EMR - EPIC Shift Info: Day 5x9-Hour 08:00 - 17:00
