About the Position

Primary Care Nurse Practitioner or Physician Assistant

Location: Indiana

Schedule: Ongoing per diem day shifts

Openings: 1

Position Overview

• Provide primary care and urgent episodic care in an outpatient clinic setting.
• Patient ages referenced as 2 and older.
• Focus includes wellness, prevention, chronic disease management, and acute episodic care.
• Occupational health and basic procedures may be required, including suturing and splinting as referenced.
• Charting system: Athena experience preferred but not required.

Required Qualifications

• Nurse Practitioner or Physician Assistant.
• Active license in Indiana.
• DEA required.
• Recent family medicine experience and ability to see pediatric patients as required by site policy.
• Minimum of two years of experience required.
• Candidates must have at least two years of clinical experience within the past five years to qualify.

Credentialing and Onboarding

• Credentialing timeline varies by site.
• Shadow training may be required prior to first shift.
