Requirements:
• Active SC license or willing to obtain
• BC/BE (If BE, within 5 yrs of eligibility)
• DEA/CSR (must be obtained prior to start)​
• Clean record
• Completed fellowship
• NPDB Self-query report (within 30 days active)
