Job Responsibilities

• Perform comprehensive exams, diagnoses, and treatment plans in an outpatient setting
• Manage acute and chronic conditions across a diverse patient population
• Order and interpret labs, imaging, and diagnostic tests
• Develop and implement patient-centered care plans
• Collaborate with specialists and refer within an integrated care network

Qualifications

• Board Certified or Board Eligible Nurse Practitioner (Family or Adult-Geriatrics)
• Active New York State NP License
• National Certification (AANP, ANCC, or ONCC)

• Bilingual (Spanish) preferred
