Join a dynamic healthcare team as a Primary Care Physician, providing comprehensive medical care across multiple settings including office-based, urgent care, house calls, and nursing homes. Focus on high-quality diagnostics, continuity of care, and collaboration with multidisciplinary healthcare teams.

Key Responsibilities:

• 🩺 Deliver primary medical care in accordance with state and national standards
• 📋 Assess, diagnose, and manage acute and chronic medical conditions
• 🧪 Order and interpret laboratory and imaging studies; refer to specialists as needed
• 💊 Prescribe medications and treatment plans, including lifestyle and dietary counseling
• 🗣️ Educate patients on treatment options and preventive care
• 📝 Maintain accurate, timely, and compliant medical documentation
• 🤝 Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• ⚡ Adhere to quality improvement, safety, and infection control standards

Requirements

• 🎓 MD or DO from an accredited U.S. medical school
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, or Palliative Care/Hospice
• Completion of an accredited U.S. residency (or equivalent pathway)
• Active New York State medical license (or eligibility during credentialing)
• DEA registration (or ability to obtain)
• Experience providing primary and diagnostic care in a clinical setting

Preferred Skills:

• Experience with geriatric or long-term care populations
• Strong collaboration and communication skills within multidisciplinary teams
