About the Position
Join a dynamic healthcare team delivering high-quality primary and diagnostic care in nursing home and long-term care settings. This role focuses on comprehensive medical management, continuity of care, and collaboration with interdisciplinary teams to optimize patient outcomes.
π° Salary Range: $200,000 β $230,000
π Key Responsibilities
- Provide primary medical care to nursing home patients in accordance with standards
- Assess, diagnose, and manage acute and chronic medical conditions
- Order and interpret laboratory and imaging studies; refer to specialists as needed
- Prescribe medications and treatment plans, including lifestyle and dietary guidance
- Educate patients on treatment options and medication side effects
- Maintain accurate, timely, and compliant documentation
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes
- Adhere to quality improvement, safety, and infection control standards
Requirements
- MD or DO from an accredited U.S. medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, or Palliative Care/Hospice
- Completion of an accredited U.S. residency (or equivalent pathway)
- Active New York State medical license (or eligibility during credentialing)
- DEA registration (or ability to obtain during credentialing)
- Experience providing primary and diagnostic care in a clinical setting
