About the Position

We are seeking an experienced Medical Director to lead a comprehensive, community-based care program focused on serving seniors with complex and chronic healthcare needs. This leadership role offers the opportunity to oversee clinical operations, drive quality outcomes, and support interdisciplinary care teams in delivering patient-centered care that promotes independence and dignity.

  • Provide medical leadership and oversight for participant care and clinical outcomes
  • Direct and support Primary Care Providers and Advanced Practice Providers
  • Oversee Quality Assurance and Performance Improvement (QAPI) initiatives
  • Develop and implement medical policies, protocols, and standards
  • Conduct comprehensive assessments including histories, physicals, and periodic reassessments
  • Collaborate with interdisciplinary teams to coordinate individualized care plans
  • Participate in intake, evaluation, and enrollment decisions for program participants
  • Lead and contribute to team meetings, case conferences, and care planning discussions
  • Educate patients and families on chronic disease management and preventive care
  • Ensure delivery of care across various settings including clinic, home, and community environments
  • Participate in an on-call rotation as required
  • Foster a collaborative, mission-driven culture focused on improving senior health outcomes

Qualification:

  • Board Certified in Internal Medicine or Family Medicine (required)
  • Board Certification in Geriatrics (preferred)
  • Active Massachusetts medical license (or ability to obtain)
  • DEA and controlled substance registration (required)
  • Minimum of 1 year experience working with frail or elderly populations
  • Prior leadership or supervisory experience (required)
  • Strong clinical skills in chronic disease management and geriatric care
  • Ability to work effectively in an interdisciplinary, team-based care model
  • Commitment to innovative, community-based healthcare delivery
  • Clean malpractice history preferred
