About the Position
An established healthcare program in Mishawaka is seeking a Medical Director specializing in Geriatric Medicine to lead a Program of All-Inclusive Care for the Elderly (PACE). This full-time, permanent position offers a unique opportunity to combine clinical care with leadership in a collaborative, mission-driven environment focused on improving outcomes for frail and elderly populations.
The Medical Director will oversee clinical operations, ensure quality patient care, and lead interdisciplinary teams in delivering comprehensive services across multiple care settings. This role involves both direct patient care and administrative leadership, including policy development, quality improvement initiatives, and provider supervision.

Core Responsibilities:
- Provide medical direction and oversight of patient care and clinical outcomes
- Lead Quality Assurance Performance Improvement (QAPI) initiatives
- Supervise and mentor physicians, APPs, and care teams
- Conduct patient assessments, develop treatment plans, and manage care
- Participate in interdisciplinary team meetings and care planning
- Engage with patients and families to support long-term care decisions
- Ensure compliance with regulatory standards and organizational policies
Qualification
- Board Certified in Family Medicine or Internal Medicine (Required)
- Fellowship training or Certification in Geriatric Medicine (Preferred/Highly Preferred)
- Active Indiana License or willingness to obtain (Required)
- Minimum 1 year of experience with frail or elderly populations (Required)
- Prior leadership or supervisory experience (Required)
- BLS, DEA, and CDS certifications (Required by start)
- Valid driver’s license and reliable transportation (Required)
- Clean malpractice history (Preferred)
- Current NPDB Self-Query report (Required)
- Must be seeking a permanent position (Required; no locums)
