Key Responsibilities:

Provide comprehensive outpatient primary care to individuals and families

Perform patient assessments, diagnosis, and treatment planning

Promote health education, wellness, and disease prevention

Collaborate closely with physicians, nurses, and healthcare staff to ensure coordinated care

Educate patients and families to support informed healthcare decisions

Maintain accurate and timely clinical documentation

Participate in continuous professional development and quality improvement initiatives
Schedule Highlights

Monday – Friday schedule

Day shift: 8:00 AM – 5:00 PM

Requirements

Master’s Degree from an accredited Adult Nurse Practitioner program – Required

Current Vermont APRN License (or ability to obtain) – Required

Certification as Adult, Family, or Pediatric Nurse Practitioner from a nationally recognized organization – Preferred

Healthcare Provider CPR Certification – Required

DEA License – Required

Ability to qualify for credentialing by medical staff

Clean malpractice history – Required

Minimum two years of nursing experience prior to advanced degree preferred

Most recent NPDB Self Query Report (within 30 days) required
