About the Position

We are actively seeking a dedicated Nurse Practitioner (NP) specializing in Primary Care to join a collaborative outpatient clinical team in a dynamic and patient-focused environment. Key Responsibilities:

  • Provide comprehensive outpatient primary care to individuals and families

  • Perform patient assessments, diagnosis, and treatment planning

  • Promote health education, wellness, and disease prevention

  • Collaborate closely with physicians, nurses, and healthcare staff to ensure coordinated care

  • Educate patients and families to support informed healthcare decisions

  • Maintain accurate and timely clinical documentation

  • Participate in continuous professional development and quality improvement initiatives

Schedule Highlights

  • Monday – Friday schedule

  • Day shift: 8:00 AM – 5:00 PM

  • No weekend requirements

Requirements

  • Master’s Degree from an accredited Adult Nurse Practitioner program – Required

  • Current Vermont APRN License (or ability to obtain) – Required

  • Certification as Adult, Family, or Pediatric Nurse Practitioner from a nationally recognized organization – Preferred

  • Healthcare Provider CPR Certification – Required

  • DEA License – Required

  • Ability to qualify for credentialing by medical staff

  • Clean malpractice history – Required

  • Minimum two years of nursing experience prior to advanced degree preferred

  • Most recent NPDB Self Query Report (within 30 days) required
