About the Position

Position Overview

We are seeking a Board-Certified Internal Medicine or Family Medicine Physician to serve as Medical Director for a geriatric-focused program in West Springfield, Massachusetts.

This leadership role centers on comprehensive, community-based care for frail and elderly patients within a PACE (Program of All-Inclusive Care for the Elderly) model. The Medical Director will oversee clinical operations, ensure high-quality patient outcomes, and supervise medical staff in a collaborative interdisciplinary environment.

Assignment Details

  • Employment Type: Locum

  • Schedule: Monday – Friday, Full-Time

  • Leadership Role: Reports to Executive Director

  • On-Call: Participation required as part of provider rotation

Practice Setting

  • Community-based geriatric care model (PACE)

  • Focus on adults 55+ requiring nursing-home-level care but living in the community

  • Interdisciplinary team approach

  • Emphasis on prevention, chronic disease management, and maintaining independence

Key Responsibilities

  • Provide medical direction and supervision of participant care

  • Oversee clinical outcomes and Quality Assurance Performance Improvement (QAPI) program

  • Develop and maintain medical policies, procedures, and standards

  • Supervise Primary Care Providers (PCPs) and Advanced Practice Providers (APPs)

  • Conduct new patient histories and physicals, reassessments, and treatment planning

  • Participate in interdisciplinary team meetings and family conferences

  • Manage chronic disease care for frail older adults

  • Order and interpret diagnostic tests and labs

  • Educate participants and families on health maintenance and care plans

  • Participate in intake evaluations and eligibility assessments

Requirements

Required Qualifications

  • Active Massachusetts Physician License – Required

  • Board Certified in Internal Medicine or Family Practice – Required

  • DEA Registration – Required

  • Previous supervisory experience – Required

  • Minimum one (1) year experience working with frail or elderly populations – Required

  • Self-Query NPDB (dated within the same month as submission) – Required

Preferred:

  • Board Certification in Geriatrics

  • Additional experience or qualifications in geriatric medicine
