Requirements

• Board Certified or Board Eligible in Neonatology
• Active California Medical License (or willingness to obtain)
• BLS and NRP certifications (as applicable)
• DEA (if required for practice)
• Clean malpractice and background history
• Current CV including complete work history
• NPI and supporting credentialing documentation
