About the Position

🌟 Full-Time Nurse Practitioner – Geriatric / PACE Care 🌟
      Location: West Springfield, Massachusetts

Join a mission-driven interdisciplinary team providing comprehensive, community-based care to frail and elderly adults. This full-time role focuses on maintaining participant independence, dignity, and quality of life through personalized care plans and coordination across the full continuum of services.

Key Responsibilities:

1. Perform comprehensive medical assessments and develop individualized care plans
2. Provide acute illness evaluation and treatment
3. Participate actively in discharge planning and home visits as needed
4. Coordinate care across PACE services and collaborate with interdisciplinary teams
5. Assist in the development and review of clinical policies and procedures
6. Participate in provider on-call rotation

Requirements

1. Board Certified Nurse Practitioner (ANCC or AANP) – Required
2. Active Massachusetts NP license – Required
3. DEA Registration – Required
4. CDS Certification – Required
5. BLS Certification – Required
6. Experience providing care to frail or elderly populations – Required
7. Ability to travel within the service area – Required
8. Prescriptive authority
