Assignments are scheduled based on provider availability and county demand, with opportunities in all but a limited number of restricted counties. Position Overview We are seeking experienced Nurse Practitioners and Physician Assistants to perform in home health and wellness assessments for geriatric patients. This is a flexible, provider driven opportunity built for independent clinicians who want control over their schedule while delivering meaningful, patient focused care. Schedule and Volume * Typical schedule: Monday through Sunday, 7:00 AM to 7:00 PM * Daily volume: 5 to 8 in home assessments per day * Availability requirement: Providers must be available at least two full days per week, including at least one weekday.
