DATES NEEDED Nurse Practitioner (NP)

• Starting as soon as possible, ongoing

• Location: Indiana
• Mon-Fri, 8am-5pm
• Setting: VA Clinic
• Will perform Disability Benefit Questionnaires with patients who are Active Duty or Veterans of our Armed Forces
• Patient Volume: Dependent on the number of requests and scheduled patients
• EMR: Web based portal used to fill out the DBQ and submitted to the VA.
• Must be comfortable working independently
• Must be Board Certified, Active IL License

• Weekly direct deposit and online work logs
